Some of the first questions that are asked when new patients come a fertility clinic in mumbai are “Should we start with artificial insemination or will we need to begin directly with IVF?” and “How many cycles will it take before we need to consider IVF?”
One of the main reasons that IUI treatment(intrauterine insemination) is typically recommended as a first treatment step is that it is often more cost-effective than other potential solutions. Most couples are concerned about the costs involved in fertility treatments and want to begin with treatments that are less expensive first. The cost of one IUI cycle depends on the fertility clinic, the fertility drugs prescribed, blood work and any necessary monitoring.
Many couples who have had trouble conceiving, often have success from IUI if:
—the female partner has been diagnosed with cervical factor infertility
—the female partner has cervical mucus problems
—the female partner has been diagnosed with an ovulation disorder and drugs that induce ovulation have not worked
—the male partner has problems with ejaculation
—issues are found with the male partner’s sperm, such as poor sperm movement (motility), poor sperm morphology or low sperm count
—to preserve fertility, the male partner has sperm frozen before any type of cancer treatment
—donor sperm is used
IUI is often the first step in treating unexplained infertility and mild endometriosis. Before the final decision is made to start an IUI cycle, the doctor will check to be certain the female partner has acceptable egg quality and one open fallopian tube. In addition, the female partner must have the ability to stimulate ovulation. The doctor will also check to see if the male partner’s semen meets all of the necessary parameters.
Infertility specialists often recommend starting with IVF for a variety of female fertility issues, including severe endometriosis and blocked fallopian tubes. IVF is also recommended when there is male factor infertility or if the male partner has had a vasectomy. Of course, the infertility specialist determines the best treatment plan on a case-by-case basis, looking at all of the factors involved. Depending on the situation, starting with IVF may actually be more cost-effective in the long run.
Some women/couples prefer to have an IUI without using any fertility medication. Studies have shown, however, that the success rate is higher when ovulation-inducing drugs are used. It is important to note that the possibility of becoming pregnant with multiples increases when fertility drugs are taken during an IUI cycle.
Oral fertility medications are usually taken by the female partner. Sometimes an injectable drug containing a follicle stimulating hormone may be prescribed. In order to trigger ovulation and the appropriate time for insemination, hCH may also be administered. Determining the timing of the IUI procedure can be done in two ways. With an ultrasound, the infertility specialist can observe the growth of the ovarian follicles; blood tests are the used to confirm the timing. Often though, an OPK (ovulation predictor kit) is used to figure out the female partner’s most fertile time. The OPK is used to identify the presence of luteinizing hormones in her urine.
For couples who start their fertility treatment with IUI, the success rate depends on several factors, including age. Per statistics from the American Society of Reproductive Medicine (ASRM), the success rate can be as high as 20% per cycle when the female partner is under the age of 35 and all protocols are used. The success rate steadily decrease to approximately 13% for women over the age of 35. If IUI is not successful after three to six cycles, it is usually recommended that the couple move on to ivf centre in mumbai.
